Personal Information We Collect

In addition, if you are a patient, we will ask you for your Rx Card Member ID, Social Security Number, or date of birth, and if you are a client company, we will ask you for the name of your company. Our primary purpose in gathering this personal information is to use it within mymatrixx.com to verify that you are a current client or patient serviced by myMatrixx, and in fulfilling your wishes. If you are a client company, we will also use your personal information to communicate with you (e.g., to send authorization e-mail messages or alerts, newsletters, and clinical alerts). If you are a patient, we presently only use your personal information to identify you as a patient, but in the future we may use such information to send you text or e-mail messages when your medications are ready for pick up, to alert you when your mail order medications have been shipped, and to provide you with clinical alerts and other myMatrixx communications. Additionally, we may use your personal information to inform you of our products or services or new or modified features of the mymatrixx.com Web site.

 

Other Information We Collect
